Physician Quality Measures

New PHCQA Physician Reporting Initiative (COMING SOON!)

The Pennsylvania Health Care Quality Alliance, under the direction of its Physician Advisory Committee, planning to launch a new initiative to publicly report quality data on primary care practices throughout the state. The first phase this voluntary initiative will leverage data that was collected as part of the Pennsylvania Chronic Care Initiative (CCI) in the Improving Performance In Practice (IPIP) database. The CCI program is based on the Wagner Chronic Care Model and the Patent-Centered Medical Home concept. As of 2011, over 170 practices serving over a 1.1 million patients across the Commonwealth of Pennsylvnaia have participated in the PA CCI program.

For practices that choose to participate, their data will be extracted from the IPIP database and PHCQA will then show select data on a consumer-friendly quality report, similar in format to its Hospital Report.

PHCQA will begin reporting six diabetes care measures on the physician report which provides a small but important snapshot of the care provided to patients with diabetes, a relatively common chronic disease.

Diabetes Care Measures

  • A1C documented within the last year
  • Most recent A1C level greater than 9.0%
  • Blood pressure management
  • LDL management and control
  • Tested for nephropathy or already under treatment
  • Received a retinal eye exam

For the initial launch, only practices who are current or past participants in the PA Chronic Care Initiative will be invited to participate. However, we will allow non-CCI physician practices to share their data with the public in future iterations of this report. If you are a part of a non-CCI practice who is interested in reporting please contact John Bulger at JBulger@geisinger.edu.

Participation in this program is completely voluntary and free of charge. Interested CCI practices can participate by faxing a signed copy of the Data Release Form to (717) 307-3406. Alternatively, forms can be mailed to the following address:
